Locations, Hours and Contact Info

Feel free to schedule your visit for a time that's convenient for you. We offer a variety of appointment times including some evenings (Tuesdays and Wednesdays) and every Saturday. We try our very best to confirm that you will keep your scheduled comprehensive eye examination appointment. This is especially true for our "prime time" appointments. Please give us at least 24 hours notice (48 hours notice for Saturdays) if you cannot keep your scheduled examination appointment. Thanks for visiting us!

About Your Visit

  • When you arrive, Yolanda will greet you. She will take your completed forms.
  • Then, Ashley will take you into our patient care area where she will gather important information about your eyes and about your overall health before you see your doctor.
  • Next, you and your doctor will work together to make sure that your eyes are healthy AND that you can see clearly. We may prescribe medicine, eyewear (glasses), contact lenses, eye surgery (like LASIK), or other approaches to help you.
  • We may escort you, next, to our large and beautiful optical boutique where Ulises or Bob will assist you to make a stylish frame selection for your new eyewear.
  • Finally, Crysta will work with you to be sure that your administrative work is all done.
  • If you have major medical insurance, then Lori will process your insurance claim so that your out-of-pocket costs are kept as low as possible.
  • Please make sure you let us know 24 - 48 hours ahead if you cannot keep your scheduled exam appointment. We will charge you a non-refundable $50 "no show" fee if you miss an evening or a Saturday comprehensive examination appointment.

Patient Forms

Please download, print and complete our "Welcome Letter " and "Medical History Questionnaire" at home so that you can take your time completing this information before you visit us. Once you've completed it, please bring it with you to our office. You should simply hand it to Yolanda when you check in. Thank you!

Facilities and Equipment

We have the most advanced diagnostic and testing equipment available in the optometric field today. This includes a digital fundus camera to help us view your retina. It also includes state-of-the-art corneal topography instrumentation to give you the most complete laser vision correction care.

Insurance Plans

We participate with VSP , Eye Med and Medical Eye Services.

Other major medical insurance carriers may cover our services for medical eye conditions like eye injuries, dry eyes, pinkeye, glaucoma, macular degeneration, and cataracts. We are currently on insurance panels for, Blue Cross P.P.O., Cigna, Health Net, Medicare, Aetna and others. Please bring your medical insurance coverage card with you to your examination. We will do our best to help you use your insurance coverage to its fullest so that you can reduce your out-of-pocket costs as much as possible.
